Recently seen a young girl brought to my clinic for checkup .She was brought by mother for a THROUGH Checkup.
Mother suspected that the daughter had Sexual Intercourse (with consent) or was molested by a friend.
On detailed history and examination, her apparent “Friend” was an equally Young boy whom she got to know over “Instagram” only 10 days ago😱.
This was followed by a brief ‘Making out session’ in a hotel room.☺
Mothers please be very alert and cautious with teenage children around.
Communication is the key for most of the adolescent problems .
Can we Avoid Smart phones till children are 18 or more.? Just a thought.
If not possible, then Keep yourself updated,
keep check on their posts and especially photos on Instagram.,checks on Snapchat etc.

We all know that this product is widely used nowadays especially on the backdrop of Swine Flu and other viral infections ,more common in rainy season.

Sanitizers mostly contain alcohol (ethyl alcohol) or antibiotics such as Triclosan.

However they need to be used judiciously.

Recent study’s show that bacteria develop resistance to these sanitizers.Also they strip of the outer protective later of skin and expose inner layers.

Our good old Soap and Water works equally well and is much safer.😉

Vaginal problems are very common and a little awareness can enable women to take care of this delicate region of their body. Vaginal infections are the most irritable and hurtful and lack of education on the same makes it difficult to prevent them; from burning and itching to unpleasant odour, most women have faced these infections at some point of their lives.

Here are few points you must know about to stay clear of these infections.

1. Vaginal infections: vagina contains bacteria and yeast which are good and maintain the health of the area. However, things like change in levels of hormones, poor hygiene, pregnancy, sexual
contact, antibiotics, disturb the balance of health, thus causing infections. Most common types of vaginal infections are: bacterial infections, yeast infections, trichomonas vaginitis. While there exist over-the-counter cure for these infections, it is best to visit your gynecologist and get the infections diagnosed & treated.

2. Cervix inflammation: infection or inflammation in the cervix is called cervicitis. It is pretty common and is influenced by a number of factors like allergies, infections or irritations – it is important to know the cause of cervicitis because in the long run, it could pose a problem with fertility. Symptoms of cervicitis include foul smelling vaginal discharge, pain during passing urine, pelvic pain, abnormal bleeding like bleeding after sexual contact or between periods. Cervicitis is treated with the help of anti-fungal medications, antibiotics and anti-viral medications depending upon the evaluation of the underlying cause.

3. Pelvic and uterine infection: infection in the uterus and pelvis is also a matter which should not be left untreated. It can present with pain lower abdomen, pain during intercourse, frequent and painful urination. The infection can be caused by various organisms which can lead to pelvic inflammatory disease which in turn can spread to the uterus, ovaries and the Fallopian tubes.

We all know that a Healthy Sex Life keeps our immune system humming, lessens pain and relieves stress. It makes for a happier life. But what happens when we #donthavesex?

Well, according to recent studies, we can become depressed – if we don’t have sex regularly. Or, more specifically, our vaginas may become #depressed.”…. Vaginas getting depressed??? Uh oh….. But yes… They do.

Having a depressed Vagina is a real condition, backed by scientific observations, reasons…. And yes… Remedies!!

Those suffering from this condition experience burning and itching sensation, without having any infection down there.

I come across such patients quite frequently.

And needless to say, this is a cause of worry. Worry, because most of these Women are young and otherwise healthy. But their frequency of sexual intercourse is abnormally less… To an extent of Non-Existent. Most of them try to attribute it to their hectic lifestyle, odd working hours of both partners, high level of stress etc.

Poor souls don’t understand what they are missing out on…. They’re missing out on something, which is one of the most beautiful feelings/acts in the world.

But the problem lies deeper down…. Pun intended 😉

Put simply, having Regular Sexual Intercourse is an important part of a Woman’s health. #Sexualabstinence can make your #vaginadepressed. And this invariably leads to #vaginalatrophy.

In Vaginal atrophy, the vaginal wall becomes thin.
*This Vaginal atrophy could be attributed to a variety of factors* some of them are as under

1. Lack of sexual intercourse.
Remember vagina is a muscle. When put to good use 😉 it helps us maintain comfort while doing our regular chores and activities ……including sex.
#Sex and #orgasm help in increased blood flow to the vaginal muscles. This is needed for the oxygen to reach the muscles and keep them hail and hearty. Just like we work out in order to remain fit.
So from this perspective Consistent sexual play either with self or a partner is helpful…
Yes… Be your own partner if you don’t have one 😉

2. Other factors that contribute to Vaginal atrophy are smoking and no vaginal births.
Yes… We can always avoid/quit smoking… For Lungs and hearts are not the only parts that get damaged…🙄
But giving no vaginal birth is out of your control. ..it’s situational

3. Reduced level of estrogen
Vaginal atrophy can affect women of any age. But it is prevalent among #PerimenopausalWomen.
As we know #Estrogen is needed to regulate the ovulation and menstrual cycle. But women experiencing menopause stop producing estrogen. And, the vaginal walls start becoming thinner and dryer… Leading to loss of interest or more so, becoming apprehensive about having sex.

There are many other effects of levels of estrogen.

A. It affects women treated for breast cancer

B. Women face a medical condition called premature ovarian failure. This happens because they stopped producing estrogen before they turned 40.

So be watchful…
“when you see thinning of the vaginal walls, a loss of rugae (which are those sensitive folds in the vagina), bleeding or loss of lubrication,”

There is a change of estrogen level in you.

*#SymptomsofVaginalatrophy* 

1. You may be experiencing Vaginal atrophy if there is #drynessirritationandsoreness. Some women may also experience a burning sensation, particularly when they pass urine.

2. There could be pain during sexual intercourse

3. Some women face frequent urination.

4. There may also be itching because of the loss of lubrication.

Now, Lack of sexual activity decreases the vaginal muscle tone. This causes the muscle to become weak due to lack of use. That’s why, some form of sexual activity with the partner (or with self) is necessary to keep the muscles functioning and healthy .

Most of these are women who haven’t had sex for a long time, for various reasons…. I do sympathize with them.

But let me show them a way out…
*#VaginalAtrophy.
Is there a way out?*
*YES*
(I would rather be showing them the way *in*😉😉)

It could be as simple as going on a lovely romantic date … Which begins with holding hands first. …. and then as the romance progresses…leave something for imagination😀

But yes… you women out there… don’t be shy to experiment with various forms of foreplay for a while. For that gives the much needed wetness or lubrication for a highly enjoyable Sexual intercourse.

And trust me… Then it doesn’t remain “Sexual Intercourse”… You graduate into… And slowly become the master of the art of “Love Making”

Wetness…. Oh yes.. wetness…

Moisturizers and lubricants go a long way in improving the tissue, and needless to say, add to the fun.

There is also massage… Vaginal massage.

#Vaginalmassage relaxes the muscles and gest the blood flowing.
Remember …In the end, it’s about exercising the tissue through various forms of sexual activities.
Having sex or engaging in any form of sexual activity on a regular basis is an important part of women’s Physical as well as Emotional well-being.
Estrogen creams and tablets are also recommended. They revitalize the tissue and restore the body’s natural hormone level.

A topic close to everybody’s heart…. And…. Genitals😉

Before one gets “seasoned”😉 and gets used to a particular contraceptive, especially “Newbies” find it difficult to make a choice amongst the various contraceptives displayed on the shelf.

The reason behind writing this is helping the “Newbies” make a better choice and helping the “Seasoned” understand their methods better.

Well… To begin with, a “good” contraceptive “must” be

1. #Safe
Safe has two meanings.
A. With minimum side effects
B. #Minimumfailure rate… i.e. to avoid “mistakes” in the dark.

2. #Easily Available
It’s a very natural and beautiful act to make love. And it should not be hindered due to unavailability of “resources”.
So make sure that your “method” is available in the shop round the corner.

3. #cheaprate
You’re smart enough to understand that by Cheap, I mean price – and not quality.

4. #Notinterfere with the sexual act.
Imagine something – itchy -scratchy-pinchy. You certainly don’t want to hear “ouch” . So avoid, if it doesn’t suit you.

Now little on a serious note, let’s look at the types.

1. #OralContraceptive Pills/ OCPs
This is the most common method used by women all over the world. However there are lots of myths and misconceptions.
Let me clear them for you…
There are various low dose pills available today. Their main advantage is, ‘Regular pain free cycles’ with ‘minimum failure rate’ . They also give clear skin and are reversible in action.
Disadvantage – No protection from STDs and HIV.

2. #CopperT/LNG-IUS(#Mirena).
What’s the Property of Copper T?
Well… These are devices which fit inside the uterus. (Ahem.. The doctor fits them)
Mainly used for spacing in between 2 children, or for long term use.(5-10 years).
This is a fairly well tolerated and reliable method of Contraception.
But again – no protection from STDs and HIV

3. #Hormonalinjectons.
In this Injections of hormones are given over regular intervals. The failure rate is 0.
These are widely used by Govt. of India on the Indo-Nepal border.
Disadvantage – Same as above points

4.#FemaleCondoms.
Not very easily available.
Difficult to use.
But …. Good protection against STDs and HIV.

4A. #MaleCondoms
Use it if you and your partner are okay with no “skin to skin” touch.
Good protection against STDs and HIV.
If someone has doubts about durability,it reminds me of a joke.
Apply some chilly powder between 2 condoms before use.If the outer one breaks he screams.
It the inner one breaks she will scream 😀😀

5. #Tubectomy/Sterilization Operation.
Important Note-
“i-pill” OR the “Morning after Pill” is NOT a contraceptive for regular use.
*It is to be used only in an Emergency.*
Remember,every Woman’s health and body type is different. There is NO ideal Contraceptive available (except Abstinence😉)
Talk to your Doctor and make an informed choice.

#Coexistance and #Communication Part 2
#Speech and #Actions

In my last write up on this subject, we had seen 4 possibilities about Coexistence and Communication – telling us how it should be.

Coming to think of it, this is a very easy method for assessing any situation.

This is called 2 dimensional analysis model. Wherein, we plot any 2 parameters on X and Y axis respectively. Then we draw 4 quadrants, and evaluate the situation as to “what it is” and “what it should be”

As we had seen in my last post
X axis had coexistence
0= Hostile Coexistence
10= Peaceful Coexistence
Y Axis had Commmunication on it
0= No Dialogue
10 = Joyful/loving Dialogues
And then we had seen the 4 possibilities arising out of it.

To Many a times I find that there is a lot of difference between the speech and actual actions of the couples.
And many a times, that is the reason for the discord.
So this time, let’s plot
Speech on X Axis
Wherein
0= insulting / hurtful/sarcastic speech
10= loving and caring speech
And Actions on Y Axis
Wherein
0= Not doing anything/or doing things without really caring about what the other partner wants
10= Doing things lovingly and willingly by understanding your partner’s mind

1.#lovingspeech and #lovingactions
As I had mentioned in my last post
“The husband and wife are really appreciative of each other’s efforts and achievements. Even if the lady is a housewife, the husband is full of praise for her as to the way she has aesthetically decorated the house – the way she lovingly caters food to everyone’s taste. And the lady also looks at her husband with starry eyes for the hard work that he puts in for the well-being of the family”

2. #loving speech and #No Actions
Either the husband or a wife is a complete sweet talker.No actions, simply words.
But either they have hidden agendas and ulterior motives, Or they’re completely lazy and want to cover up by sweet talking.
If the partner is tolerant of such behaviour, well and good, else this might lead to remorseful feelings such as “wasting your life for wrong people”

3.# Harsh Speech and #Loving Actions
I have seen such interactions mostly between a Mother in Law and a Daughter in law.
Many a times, the husband is posted somewhere out of station, and the parents of the pregnant wife are settled in some far away small town. So, the Mother in Law swings into action, religiously accompanying the Daughter in Law. Making sure that care is being taken and all the instructions that I give are being followed. Their speech might be a bit stern – but the affection in their actions is beyond words.

4.#Harsh Speech #Hostile Actions
Do I really need to explain this?
All I can say is, after the doctor’s visit, they are shortly going to need to visit the lawyer as well.

MID LIFE SEX.

I often come across women who come to my clinic with various complaints related to their periods or gynaecology issues.On deeper probing these are few of their commonest complaints.

1.I rarely feel in the mood to have Sex lately.
2. I don’t have any spontaneous desire.
3.Nothing seems to excite me like before.
4. I don’t remember the last time we had Sex!
5.I am always looking for an excuse to avoid Sex.

Now before you start relating yourself to this, always remember…. This is Normal as you age……

But this can Change easily.🙂
To help overcome this, let’s Understand the Sexual Cycle in short .
Basically, Women’s sexual desire is different from Men.

Women’s desire is more often responsive rather than spontaneous and can be very sensitive to the context. What that means is, if the relationship is in good shape then your body and mind will be more likely to respond to loving touch and desire will be created by that caress or hug; even if it does takes a while.

Suppressed Feelings.

What that also means is that if you aren’t happy with your partner and you can’t talk about things, then a hug or caress can feel the opposite, irritating or even downright repulsive! Often your body’s response to your partner is a sign that you need to spend more quality time or go back to that disappointment or hurt feelings that everyone is avoiding.

Gender Differences

Men’s desire is mostly, but not always spontaneous, sexual thoughts just pop into their minds anywhere and any time🙄.

It can also be assumed that a man with an erection is turned on and ready; that the body matches the mind. This can be true also for women but there’s far more variety. A woman’s genitals (being wet or dry) do not necessarily match the mental arousal. Women’s arousal is far more subjective and about the context again, whereas men’s desire is far less negatively impacted by what’s going on in his life!

However, stress, tiredness or an ongoing upset can all reduce a woman’s sexual desire to a great extent.
“Your sexuality is unique”.
We plan everything else that’s important to us; a night watching our favourite movie, going to a popular restaurant and seeing friends so what’s going on with the idea that you don’t need to plan time for intimacy with your partner.
Of course you do!!

Doctor … She’s my friend “Mrs. X” ….wanted to show her reports to you. Basically, we need 2nd opinion.

Me… Please have a seat…. Please tell me what the exact problem is… I’ll see the reports after that.

Mrs. X.(age 50). :
Nothing much madam… I’m bleeding on and off. Not much – but little bit. But there’s absolutely no pain what so ever in my abdomen. I’m easily able to run my daily errands….Nor do I have any weakness.. I guess, these people are over-thinking… Worrying too much unnecessarily.

In fact, I stopped having my periods almost an year ago.
For that matter, I don’t even remember entering a hospital other than at the time of my delivery.

Well… After the check up, I saw the sonography report. And I asked her to get it done again from a senior sinologist.

The next day Mrs. X arrived jubilantly with the fresh report….
“The sonologist said that there was nothing to worry about. …and that madam will explain to you the details 🙂… He’s such a nice doctor”

Me : Well…. I beg to differ. My opinion is a little different. I think, we should go in for a biopsy. Then only , we can be 100% sure about the diagnosis.

Mrs. X: Madam… It’s wedding season. I own a beauty parlour. I have too many orders pending… What should I do?🙄.

Me: My sincere advice to you is, you should get the biopsy done ASAP… Without any further delay.. and once it shows that the reports are normal, then you can resume work immediately.

Well… The biopsy was performed. The reports came in after 3 days.

We planned the surgery immediately… Without further delay.
The patient is now handling the parlour brilliantly well, just within a month!!!
This is real life story of a lady from Pune who has overcome Cancer and is a true fighter!💐

Name withheld to protect her identity.

Note : The lady didn’t have any other symptoms other than sporadic – irregular bleeding. Neither did the girth of her abdomen increased, nor did she lose weight, and there was no abdominal pain or discomfort what so ever.

That’s why it is so very important to get yourself screened and checked up regularly.
A stitch in time – saves nine!!!

A lady with a few anxious relatives comes to the Opd with a Sonography report. The report mentions a tumour in the Uterus (fibroid).

Now any tumour in the body always give rise to a chain of reactions.The anxiety and fear is about the possibility of malignancy or the dreaded ‘C’ word or CANCER which almost all fear to even utter .

So now let us see what is this Fibroid.

This is a fibrous non cancerous tumour which develops from the muscles of the Uterus

Fibroids generally are not very troublesome.they can be in the uterus for years together.These are hormone (Estrogen) dependant and so can increase in size in the reproductive years, and also during
Pregnancy.Their size tends to reduce as the lady approaches Menopause .This size can range from a pea to a watermelon.

About 10% of all women have Fibroids.

Fibriods can arise from different sites inside the Uterus.If they are inside the cavity of the Uterus,they usually cause heavy and painful periods and so are diagnosed and treated early.

Fibroids which arise from the muscle wall or outside the uterus usually don’t cause any symptoms.Such fibroids are usually reported on sonography done for some other problems or on routine screening.

If the fibroids are not causing any problems, they rarely require treatment.

Here is where Counselling the patient and removing all the fear from her mind plays an important role.

There are various medical treatment available for fibroids.The LNG IUS/Mirena is a very good choice .It helps to reduce pain and heavy bleeding.In addition,it works an an excellent Contraceptive.The fibroid can be removed surgically ,if the uterus has to be retained for fertility.

If child bearing is complete, Hysterectomy or removal of the Uterus gives a complete cure.

Fibroids of the Uterus are one of the most common tumours occurring in women of reproductive age .They usually cause heavy and painful periods which can cause a drop in haemoglobin and it’s after effects.I see quite a few patients in my clinical practice who have been advised to go for surgical treatment to remove these fibroids .
The treatment of fibroids depends on the symptoms it is causing . Many a times the size of the fibriod does not corelate with the problem it causes.Sometimes Fibroids as small as 2 to 4 cm can cause heavy bleeding, on the otherhand we find Fibroids as large as a Pomegranate or a Musk Melon on routine Sonography where the lady has absolutely no symtoms.

There are various medical treatment options(No Surgery)now available for the treatment of Fibriods.

These tumours almost NEVER Develop into CANCER, So no need to panic.I would like to quote an interesting story of a patient I operated sometimes back.She came to me for persistent weight gain which refused to budge even after intense Diet and lifestyle changes. She was mainly concerned about her abdominal fat which refused to reduce. She joined a Gym but couldn’t do rigorous exercises.

On examination,i found a HUGE fibriod,for which Surgery was the only option.
The Fibroid was weighed after removal and was a whooping 6.5kg.one
So it’s a good idea to get yourself checked by a Professional before becoming obsessed with your body image and trying all sorts of treatments without expert advice.
